Program managers, system owners, and survey/form owners should use this mandatory reference as a guide to craft the required components of a Privacy Act Section (e)(3) Statement or Notice (PA Statement). This template is used to gather information from program managers and system owners in order to create a PA Statement for a survey or form used by the program or system.
