USAID’s Leadership to Sustain and Accelerate the HIV Response

PEPFAR launched its latest five-year strategy in 2022, Fulfilling America's Promise to End the HIV/AIDS Pandemic by 2030. Sustaining the gains of the global HIV response is one of five strategic pillars. This pillar underscores the importance of global, regional, and country-level planning for sustainable HIV responses beyond 2030, and states that maintaining treatment continuity, reducing mortality, reducing new HIV infections, and improving quality of life are critical components of the sustained response.

In early 2024, UNAIDS launched a “new sustainability approach” to the global HIV response, noting that planning is urgently needed for sustaining the gains from the HIV response beyond 2030.

USAID is committed to implementing the PEPFAR strategy and supports the UNAIDS-facilitated effort to transform the global HIV response to one where countries lead in achieving and maintaining epidemic control. On this journey to accelerating and sustaining the impact of HIV responses globally, we envision a country-led effort that centers diverse populations, communities, health workforce leadership, and all voices that participate in planning and decision-making.

Sustainability can only be achieved when we close the gaps to equitable treatment and services for all populations, ensuring country-led, quality, person-centered care for all.

Our Approach

USAID’s leadership for a sustainable, global HIV response leverages our unique role as the comprehensive development partner within PEPFAR. We advance vibrant and innovative partnerships in education, economic growth, democracy and governance, health, humanitarian assistance, and human rights. When leveraged, these resources, expertise, and approaches can help achieve long-term sustainability and success in the decades-long journey to control the HIV epidemic.

USAID brings a holistic development approach and diverse expertise to the sustainability of the HIV response. Our workforce has the skills and experience to address challenges across the entire health and development landscape. With this diverse health expertise comes the capacity to transform systems; to remove key barriers in governance, financial management, and human rights; and to leverage education, youth development, and economic empowerment for sustained HIV epidemic control.

Taking into account this diverse expertise, USAID has identified six HIV-focused core priorities that are critical to achieve a sustainable HIV response by and beyond 2030:
